OUR AGENCY
Healthy Care Horizons is a home care agency that is committed to providing high quality, client-centered and affordable home care services to our clients to assist them to lead dignified and independent lives in the comfort and safety of their own homes. This includes bathing, dressing, eating, mobility, toileting and physical and emotional support that is needed to enhance the clients' medical condition. The quality of care shall be monitored with care plans, outcomes and personnel credentials. We currently service Durham, Wake, Orange, Guilford, Granville, Person and Vance counties. We currently accept Medicaid, WellCare, Carolina Complete, Healthy Blue and private pay.

Our Services

Personal Care Services
This service provides daily personal care needs that include assistance with toileting, bathing, dressing, mobility, eating, meal preparation and set up, self administration of medication, home care and safety management. This service is $22 per hour with a minimum of 4 hours per week.

Respite Care
This service provides temporary relief to caregivers providing care to their loved ones. This services ranges from $20-$25 per hour depeneding on the care needed.

Companion/Sitter
This service provides emotional support and socialization skills enhancement to those in need. Normally for older adults and people with disabilities. This service is $18 per hour with a minimum of 2 hours per visit.
Prices are subject to change based on needs of the client and if care is needed on holidays
